CLAIMS PROCEDURE

Notice of Injury or Death – Written notice of the injury on which claim must be given to the Company within (20) days after the date of accident. In case of Accidental Death, immediate notice thereof must be given to the Company. Failure to give notice within the time provided in the policy shall not invalidate any claim. If it can be shown that it was not reasonably possible to do so.

  • Age Eligibility – 1-99 years Old
  • Maximum of Five (5) card per person

CLAIM GUIDELINES AND SUPPORTING DOCUMENTS
Basic required documents
For death claims:

1. Death certificate (original or certified true copy from the original duly signed by the local civil registrar)
*If death certificate is photocopy, it should be originally certified by the local civil registrar of the place where death occurred with official seal and bearing the local civil registry number indicated on the upper right side of the death certificate

2. Proof of Beneficiary/ies:

2.1 If declared beneficiary is the spouse – Marriage contract (photocopy will do)
2.2 If declared beneficiaries are children:

2.2.a. if children are below 18 years old:

1. Birth certificates or Baptismal Certificates of children (photocopy)

2. Affidavit of Guardianship duly notarized (original copy)

2.2.b. for married daughters:

1. Marriage contract (to establish the use of another family name)
2.2.c. for children who are of legal age and still single:
1. Birth or baptismal certificates (photocopy will do)
2.3 If deceased is single

*If parents of the deceased are still alive, the proceeds of the insurance will be automatically granted to them. We will then require the Birth certificate or Baptismal Certificate of the deceased.
* If parents of the deceased are already dead, the next in line who is entitled as beneficiary/ies is/are the siblings of the deceased. The following documents will then be required:

a. Birth or baptismal certificate of deceased
b. Birth or baptismal certificate of all single siblings (brothers/sisters)
c. Marriage contract of all married sisters

Others when required/needed (case to case):

  • Police or Accident Report (when by violent death or accident)
  • Autopsy Report or Post Mortem Examination Report (if autopsy were undertaken)
  • Affidavit of guardianship (of guardian if declared beneficiaries were minors)
  • Sworn Statement/Affidavit of eye-witness/es (if any in medico legal cases)
