Claim Requirements MUDAS Coverage:
- Fill-out three (3) copies of identification and claimant statement forms.
- Certified true copy of Death Certificate.
- Photocopy of Marriage Certificate, if married. Birth Certificate, if single.
- Photocopy of valid ID of the claimant.
- Provide waiver of rights if claimant is not the designated beneficiary.
If Accidental death, add:
- Police report/incident report
Claim Requirements for Hospitalization:
- Original or certified true copy of hospital billing
- Original or certified true copy of medical certificate
- Photocopy of valid ID of the member.
- Photocopy of Marriage Certificate, if married. Birth Certificate, if single.
