LOCAL MEMBERSHIP

1. Attend Pre-Membership Education Seminar (PMES) through:
   1. Online PMES request (Visit Online PMES Request)
   2. Online PMES Video (Visit Online PMES Video)
   3. On-field through an account officer
   4. At the branch, from Monday to Saturday 3:00 P.M. to 4:00 P.M.

   And received a PMES certificate valid for six (6) months only.

2. Pay the required initial fees:

   2,000               –           Share Capital (initial)

   500                  –           Regular Savings

   1,280               –           MUDAS

   200                  –           Membership Fee

   3,980.00                      TOTAL

3. Submit and present the following documents:
   1. At least one (1) primary Valid ID and or two (2) secondary IDs

      Primary IDs are:

   • Tax Identification Number (TIN)
   • SSS/GSIS ID
   • Driver’s License ID
   • PRC License ID
   • Passport
   • Digital Comelec ID
   • Senior Citizen ID
   • New Postal ID

   Secondary IDs are:

   • Pag-ibig ID
   • Philhealth ID
   • School ID
   • Company ID
   2. 2 pieces 2×2 recent photo preferably white background
   3. Birth certificate, if single
   4. Marriage certificate, if married
